Hypoparathyroidism, hypocalcemia and bone density?

One of the side effects of chronic hypocalcemia is loss of bone density. However, if hypocalcemia is caused by hypoparathyroidism would bone density be increased or not affected. If osteoclasts are not stimulated, then bone matrix can not be demineralized, right? If hypoparathyroidism is caused by a total thyroidectomy then would bone density remain stable? This is assuming the hypoparathyroidsim is permanent and not transient.

How is permanent hypoparathyroidism after thyroidectomy treated? I understand calcium and vitamin D supplements will be a part of treatment but what about calcitonin? Isn't bone remodeling important to the strength of muscle attachment sites? Is a patient with Iatrogenic Hypoparathyroidism at increase risk for muscle strains?

Any info would be appreciated.
